New Biomedical Consent Form Template

Dear Northwestern Research Community,

The Biomedical Consent Form Template (HRP-529) has been updated to version 12-03-2021. The revisions include more detailed instructions, and minor changes to template language. The document can be found under Resources & Guidance, within the Consent Templates section of the website.

Please utilize this version of the template for any new projects. At the time of any modification involving consent form updates, please consider making any relevant revisions from the updated template.
